We all know the importance of physical exercise, but it’s also important to keep your mind active. This lockdown is not for your mind. When stuck at home as a result of the coronavirus outbreak, there are chances are people are looking out for something to do. Solving jigsaw puzzles and playing video games can be fun, to get through a few hours of the day; there are some other ways to pass your time in lockdown more effectively. Be it a new skill, which you always wanted to learn, or a hobby you want to pick back up again. Now it is the perfect time to be busy with yourself and make the time in self-isolation much more worthwhile.

Learning a New Language

Many of us have an urge to learn a new language, but never get around or don’t get time for that. So, its the time to break the mould and learn it for yourself. Choose a language – may it be French, Spanish, Mandarin, Italian or Hebrew, it can make a big difference in your life once we finally get through the coronavirus pandemic. While travelling to a country and being able to speak the language of that country, will help you get engaged in more meaningful conversations with the locals and at the same time it will make you more employable.

Write Diaries

We are living in a different time. Humans have not faced such a crisis like this during the past. Write down the experiences and feelings going on in the heart and mind about the ongoing pandemic in a diary.

Skill-up with an Online Course

Many universities are offering free online courses ranging from public speaking to blockchain technology and almost everything. There are several other platforms that have high-quality videos on various topics. There are many other online learning websites that are chargeable for the courses and it is worth the time and money. Get back to your office with new skill-sets that will be helpful for you in the future.

Stay Connected

Learning life skills during the lockdown

Healthy relationships are the key to our well-being. Research suggests that people who engage in supportive and positive relationships produce more oxytocin, which can: boost the immune system, allows healing quickly and are less likely to experience stress and depression.

Technology may be blamed to make people isolated but now it is vital in building a sense of real-life community that we are missing at the moment. Speaking to at least one person can uplift you (phone or video call) every day, like sharing meals, play online games or having creative sessions and discussions with loved ones using apps like Zoom or Google Meet.

Be Mindful

During these difficult times, it’s easy for our mind to deviate towards the worries and the worst scenarios. The right thing is that no one really knows what the future holds. We should be prepared for it and the foremost thing is we are safe now in the present moment. Meditation is the only way for staying mindful and helps in keeping our focus during other tasks.

Read a Book

Keyhole surgery in newborns https://omnihospitals.in/keyhole-surgery-in-newborns/ https://omnihospitals.in/keyhole-surgery-in-newborns/#respond Thu, 08 Aug 2019 06:09:57 +0000 http://omnihospitals.in/?p=9517 Keyhole surgery in newborns

Laparoscopic surgery is a minimally invasive procedure that involves small keyhole incisions made on the stomach resulting in shorter recovery time, reduced chance of early and late potential complications and improved cosmetic results in comparison to the traditional open surgery approach. Here is an excerpt from a video interview of Dr M. Srinivasa Rao, Chief Paediatric Surgeon at Omni RK Hospitals, Visakhapatnam on Keyhole surgeries in newborns and common queries related to open surgery and keyhole surgery.

Q. What is keyhole surgery? Explain the difference between a keyhole surgery in paediatrics and newborns.

A. Keyhole surgery is one in which the surgery is performed without cutting open the abdomen or chest. It is performed using long instruments which are inserted into the abdomen. This surgery differs between paediatrics and newborns in a way that newborns are very small and are less than or equal to 28 days and weigh less than 5 kgs while paediatric surgeries are performed on children above the age of 1 year and weigh more than 5 kgs.

Q. As a paediatric surgeon, what are the advantages of performing surgeries on newborns?

A. Many newborns are born with medical conditions, small and big, like Hyaline membrane disease (HMD) and Macrophage activation syndrome (MAS). Some newborns also come to the hospital with deformities and small intestine obstruction, large intestine obstruction etc. In such cases, the newborns need to be operated on, as soon as possible, otherwise, there is a high chance of mortality in these babies. So a lot of precautions are taken from our end to diagnose and treat the baby as soon as possible to help him/her survive. This makes most newborn surgeries emergency centric.

Q. What surgeries are generally performed in newborns?

A. Generally, most commonly performed surgeries on newborns are, 1. Laparoscopic surgeries for hernia 2. Abdominal surgeries 3. Small bowel obstructions Although not all babies are suitable for laparoscopic surgery, most babies can be stabilised.

Q. What are the recent advancements in surgeries performed on newborns?

A. There is quite some development in the surgeries performed on newborns in the last five years. Most significant developments are in the areas of, 1. Antibiotics 2. Anaesthetic agents 3. NICU care Due to these advancements in science and health care unit facilities, the mortality rate of babies has come down to single-digit numbers.

Q. What are the different medical and surgical precautions do doctors take while treating newborns?

A. When babies are born prematurely and weigh just about 500-700 gms, then special care is given to them and they are only taken care of by neonatologists. The nursing unit takes care of these babies for 2 months until their weight is regularised to 1.8kgs. Once the baby reaches 1.8kgs, they are safe to be taken home.

Q. What are the special units in which newborns are operated?

A. Nowadays, there are special units to operate on newborns. These units consist of advanced equipment and technology which makes the surgical procedures easier. It also consists of advanced specialists and dedicated teams to take care of newborns. Many such units have opened up in India in the recent past, in government as well as private hospitals. Due to these advancements in science and health care unit facilities, the mortality rate of babies has come down to single-digit numbers.

Q. What special care do parents have to take care of before surgery?

A. There are no specific care or precautions that parents have to take before the surgery. They just have to be able to notice the problem area in the baby as soon as possible and consult a doctor at the earliest.

Q. Are there any side-effects of endoscopic surgery?

A. Side-effects of endoscopic surgery are equal to that of open surgery. The complications that occur and in open surgery are also seen in endoscopic surgery. For example, when an intestine is joined, bleeding occurs if it an open surgery or endoscopic surgery. Hence, there are no special side effects that are seen in endoscopic surgery that are different from open surgery. Minimally invasive surgeries require more expertise and precision from the doctor’s end.

Q. Is a keyhole surgery required for all babies?

A. If the baby is stable and is not suffering from any medical conditions like bleeding, then a keyhole surgery cannot be performed. Although, if a baby is suffering from infections or any other deformities, then a keyhole surgery will be required.

Q. What are the different conditions that can be treated using keyhole surgery?

A. Different conditions that can be treated using keyhole surgery are: 1. Laparoscopic surgeries for hernia 2. Abdominal surgeries and 3. Small bowel obstructions 4. Cyst surgeries (chest and abdomen) Although not all babies are suitable for laparoscopic surgery, most babies can be stabilised.

Q. What are the advantages of keyhole surgery?

A. The advantages of keyhole surgery are: 1. The operation is done under good lighting and magnification. Hence, the chances for errors are greatly reduced during surgical procedures. 2. Laparoscopy helps in reaching inaccessible parts of the body with ease due to use of long instruments. 3. The dissection and suturing part of the surgery is also easier in keyhole surgery. 4. Operation on newborns for the second time can be done easily due to the minimally invasive procedure. Hence, in keyhole surgery, there is less scarring and the healing is quicker when compared to open surgery.

Q. What happens in a minimally invasive surgery?

A. Keyhole surgery is one in which the surgery is performed without cutting open the abdomen or chest. It is performed using long instruments which are inserted into the abdomen. Different conditions that can be treated using keyhole surgery are: 1. Laparoscopic surgeries for hernia 2. Abdominal surgeries 3. Small bowel obstructions 4. Cyst surgeries (chest and abdomen)

Q. Are there any alternatives to keyhole surgery?

A. Depending on the condition of the baby, an open surgery/conventional surgery can be performed on the baby. In some cases, the baby doesn’t even require surgery. Minimal medical treatment is enough in some babies.

Q. What special care do parents have to take care of before and after keyhole surgery?

A. There are no specific care or precautions that parents have to take before the surgery. They just have to be able to notice the problem area in the baby as soon as possible and consult a doctor at the earliest. After the surgery, a follow-up will be required to check on the baby’s healing process.

Q. When can the baby return home?

A. A baby who has undergone a keyhole surgery can be taken home once his/her health stabilises.

Q. How long does the surgery take to heal?

A. As laparoscopic surgery is a minimally invasive procedure that involves small keyhole incisions made on a certain body part resulting in shorter recovery time.]]>

In conversation with Dr Raghava Dutt Mulukutla, Orthopedic Surgeon and Chief of spine surgery

Q. Doctor I am suffering from back and leg pain and I am told that my vertebra has slipped forward.

A. You are suffering from a condition called spondylolisthesis. Here one of the vertebra slips forwards over the vertebra below.

Q. How does this happen?

A. There are a number of reasons for this. This could result from anomalies in the spine at birth, some in early childhood and some due to fractures and various diseases and conditions of spine.

The two most common varieties

1. the first variety, when the vertebra slips forwards between 4-6 years of age. This may cause back pain or leg pain during childhood or during adulthood. Sometimes due to extra weight of pregnancy, women come to doctors with back pain and this condition is then diagnosed

2. the second variety is one which is somewhat common in females at about the age of forty. This is due to degeneration (wear and tear) and is more common in women who are overweight.

Q. What are the treatment options for this condition

A. Most patients get better with physiotherapy and medication. Once the pain subsides it is important to start exercise programmes to strengthen back and abdominal muscle. Walking, sports, yoga, swimming all help.

Q. Do I require surgery?

A. Only if the pain does not subside with physiotherapy, restricted activity for a few weeks followed by exercises etc. Surgery is more beneficial in those who suffer from leg pain rather than back pain alone

Q. What is the type of surgery that is done?

A. In children and young adults sometimes repair of the defect in the vertebra .is done to prevent back pain.Mostly screws and rods are placed in the spine and the vertebra that has moved forwarded is brought back to its original position. Cages are also used to maintain the reduction and restore the height between the vertebrae. Fusion is routinely done .

Q. How long do I need to take rest? And what are the precautions to be taken after surgery

A. You will be out of bed 2nd or 3rd day after surgery. You need to stay in hospital for 4-5 days. You will be in ICU for a day. You will be given a brace to support your back for a few weeks.

Q. When can I get back to work?

A. You need 6 weeks time for light duties and 3 months for heavy work.

Q. Do I need to undergo lot of physiotherapy?

A. You hardly require any physio. A few months after surgery physios will teach you a set of exercises which are very easy to follow and can be done at home.

Q. How painful is the procedure?

A. The procedure is not that painful and you will be given adequate pain relief medication after surgery

Q. can I play games and sports after surgery?

A. Swimming, yoga ,sports are all beneficial. You will be told how to lift weights and also given a set of exercises to strengthen your back.

Q. Will surgery affect my married life?

A. Not at all . You can have a normal married life and women can have babies and undergo normal deliveries
